Physical
Information
Physical examinations are
required
for
students participating in interscholastic sports. Proof of physicals
must be presented before
students
may
practice.
There are
TWO OPTIONS for a sports physical, it can be done:
For
students who participate in more than one sport during a year:
- After the physical
for the first sport, only a
recertification form is required for students who
participate in additional sports throughout the year.

SCHOOL SPORTS PHYSICALS
Physicals for participants in all sports will be held in the High School East
Building Health Office by the school physician, Dr. Anthony
Marinello, on
dates TBA each season.
There is one date for
Winter and one date for Spring sports physicals. There will be no make-up dates.
For Fall sports we typically schedule 3 dates in the month of
August.
Physicals are open to ALL students (girls and boys in grades 7-12).
Physical
examinations are required for students participating in
interscholastic sports. Proof of physicals must be presented BEFORE
a student may tryout for a sport.

The Shenendehowa nurses have compiled
this to help you so that when you get to us, you have everything you
need. We recommend you have your parent with you or available
as one of the forms requires a parent signature.

CANDIDATE HEALTH HISTORY - HPE 6.
This form has a
front & back with 53 questions. Complete this form with your
parent within 2 days of coming to the school sports
physical! It requires a parent signature! This
form must be presented in order for the school physician to
do a school Sports Physical.
PLEASE ADD YOUR
SPORT NEXT TO YOUR BIRTHDATE.

EXAM FORM (HA1/HPE5)
This form will
be completed by the school district physician. Please only fill
out the first line: your name {last name first},
school & grade you will be attending in the Fall, & the SPORT.

MEDICATION FORM - HM2 -
DO I NEED TO
HAVE THIS? YES, if you will have any medication in the nurse’s
office OR if you plan to carry ANY medication during the
school year! For example, ANY MEDICATION,
an Inhaler, EPI PEN, prescribed medication or ANY over the counter
medication. This form MUST be completed by your physician &
parent. Please have your doctor check
off that you may carry this medication if you will be doing so.
The form should be completed by the doctor AFTER July 1st each
year.
These three forms are the only
ones you will need. Do not bring other forms with you.
It will save
time when you get to the nurses. Thanks!

PRIVATE PHYSICAL CLEARANCE
Private
physicals must be approved by the school nurse before a student can
practice.
Coaches
cannot approve or accept private physical forms.
Private
sport physicals can not be submitted during school sport physical
nights.
What

SPORTS
PHYSICAL
EXAM FORM (HA1/HPE5). The
physical must be done on a Shenendehowa School/Sports Physical
form. You must give us the ORIGINAL, complete physical, signed,
dated & stamped with the doctor’s office stamp & phone number on
it. The physical is valid for 12 months from the date of
completion.

THE
RECERTIFICATION FORM - HPE 7
- WHEN DO I USE
IT?
Complete this form ONLY IF your physical
has been done BEFORE October 2, 2009.
This form MUST be completed IN ADDITION to the
Health History (HPE6) & the physical (HPE5). This form has 16
questions. It requires a parent signature. This form MUST BE DATED
WITHIN 1 DAY of coming to the CLEARANCE hours.

STUDENTS WITH ASTHMA
Students
must have a completed the
Medication
Administered in School Form (HM2)
from their private physician with the completed
Health
History Form (HPE6) in order to attend a school physical or to
be cleared with a private physical.
In
accordance with New York State Education Law (see below), all
student athletes must have a completed form by their own physician
to self-administer any medication { i.e. Albuterol Inhaler} before
they will be allowed to participate in any school sponsored sports.
The completed form must be given to the school nurse before the
student will be placed on the clearance list.
New York State
Law Article 19- Medical and Health Services:
S916. Pupils afflicted with asthma. The board of education or
trustees of each school district and board of cooperative
educational services shall allow pupils who have been diagnosed by a
physician or other duly authorized health care provider with a
severe asthmatic condition to carry and use a prescribed inhaler
during the school day with the written permission of a physician or
other duly authorized health care provider, and parental consent,
based on such physician's or provider's determination that such
pupil is subject to sudden asthmatic attacks severe enough to
debilitate such pupil. A record of such permissions shall be
maintained in the school office. |

SELECTIVE CLASSIFICATION
FOR STUDENTS IN 7th or 8th GRADE
Selective classification
is required
for 7th or 8th graders to participate on an interscholastic
athletic team
The Selection/Classification
Program is a process for screening students to determine their
readiness to compete in interscholastic athletic competition
by evaluating their physical maturity, fitness and skill. The
student may be placed at the appropriate level of competition
which should result in increased opportunity, a fairer
competitive environment, reduced injury and greater personal
satisfaction. The intent of this program is to provide for
students in grades 7 through 12 to safely participate at an
appropriate level of competition based upon readiness rather
than age and grade.
Prior to tryouts,
students will have to complete the following:
Step 1: Student must
have a Sports Physical, either at school during the
predetermined sports physical nights (click
here for information on school physicals), or by the
student's primary medical doctor (click
here for information on private physical clearance).
Step 2: Student must
have a MATURATION screening done by the school's physicians,
Dr. Marinello or Dr. Cotugno. STUDENTS MUST COME TO A SPORTS
PHYSICAL NIGHT TO HAVE THIS DONE.
Step 3: Student must
have a Physical Fitness test done by Coaches Eggleston/Dochat,
to determine a student's ability to participate in upper level
sports.
If the student meets
all the requirements, he/she will be eligible to TRYOUT
for his/her particular sport. This process does not
guarantee participation on a team. |
